AFTER THE SUCCESS OF LMS1 AND LMS2, DAZZLER HAS CHANGED THE FORMAT FOR BIG EASTER PAYOUT IN LMS3. GET IN NOW!

This is your entry form for LMS 3 giving all the details of the competition starting on FEBRUARY 4

The form requires you to fill in your Name, Telephone No and e-mail and asks to complete your first 6 winners - those on the 4th, 11th and 25th February and 3rd, 10th and 24th March.

Again a massive 50% of the pot goes to the winner.
